Please use this page to upload all required documents for the Africa Missions Trip.
All of the following documents are required before you can travel with us:
Passport Copy
Flight Itinerary
Medical/Travel Certificate
Signed Waiver
Please make sure you have clicked on “Upload File” after you have selected your file and typed in your full name for each section. When you are finished uploading each file, you should see a green box underneath each section with: “[name_of_file] uploaded successfully” written in it. If you do not see a green box underneath each section, please go back and double-check that you have selected the file you want to upload, typed in your full name in the box provided, and clicked on “Upload File” for each section.
